Private rehab in Texas starts with Form 3207, Chemical Dependency Treatment Facility License Application. Form 3207 is used to apply for a new or change in status license for a chemical dependency treatment facility. An initial packet is a completed Form 3207, the license fee, completed Form 3208, Chemical Dependency Treatment Facility License Application Checklist for Initial Applicants, and supplemental documentation as required by the license-required and new-licensure-application rules.
Private rehab facility
Per the Form 3207 instructions, the facility must have a license for each physical location where the facility provides residential or outpatient services. Chemical dependency treatment facility licenses are non-transferable and do not have a change of ownership application process. When a new entity purchases an existing licensed facility and the purchase affects the legal name and Federal Tax ID, the existing licensed provider must submit a closure form and relinquish the license. The new entity must apply as a new applicant and go through the initial application process. HHSC will process a change in address (site move) as a new site. The legal name and Federal Employment Identification Number on the application must reflect the legal name and FEIN as filed with the Internal Revenue Service. The assumed name or doing-business-as name must reflect exactly as filed with the Texas Secretary of State's Office or the applicable county clerk's office, or both. If the physical location will be shared with non-substance use disorder treatment programs or services, HHSC asks for a complete listing of any and all non-substance use disorder treatment services or programs provided in that facility location.
How much does private rehab cost
26 TAC 564.408 sets the licensure fees a facility shall pay for any licensure period during which it provides chemical dependency treatment. Fees shall be paid in full by cashier's check, or money order. The schedule is a base fee for initial and renewal licenses, which includes application and review fees, of $1,200; a fee for each outpatient or residential site located at a separate physical address of $125; and a fee per bed of $35. A $25 fee is charged for a printed list of licensed facilities, a set of mailing labels for licensed facilities, or a replacement certificate. Licensure fees are not refundable. An application under Texas Health and Safety Code Chapter 464 and 26 TAC Chapter 564 is for licensure as a chemical dependency treatment facility only.
Private rehab treatment
Health and Safety Code Chapter 164, Treatment Facilities Marketing and Admission Practices, states its purpose is to safeguard the public against fraud, deceit, and misleading marketing practices. Nothing in that chapter should be construed to prohibit a chemical dependency facility from advertising its services in a general way or promoting its specialized services.
